• About BaptistCare Morven Gardens Centre

Situated in the beautiful mountain village of Leura in the stunning Blue Mountains, Morven Gardens features 71 beds and is a welcoming and home-like environment with lush, landscaped gardens.

• Baptistcare Morven Gardens is seeking a Permanent Part time AN- AAC Coordinator for 28 hours a fortnight.
• About the role
• To provide coordination of the AN ACC process, to lead, conduct pre-admission and complete a suite of assessments supporting the AN ACC Assessment tools.
• To ensure optimal classification and funding for BaptistCare.
• The position will ensure that BaptistCare pre-admission processes are followed.
• To coordinate and facilitate ongoing review and request for reassessment when resident's condition has changed significantly.
• To complete any supplemental funding claims to both, maximize and risk manage the home's government funding, in consultation with Funding Consultant and Client Liaison Team to achieve and maintain budget funding targets.
• To ensure thorough review and audit of Services Plus.

To enable BaptistCare Aged Care Services to meet clients' needs and the quality of service is recognized as setting industry standards.

• Some of the duties include
• Plan, coordinate and conduct the resident pre-admission.
• Ensure data entered into People Point is accurate.
• Ensure assessments are accurate, appropriate, and timely.
• Review documentation to ensure congruence. Consider both electric and hard copy documentation.
• Compile and follow the AN ACC Checklist as per Framework and Reference Manua
• About You
• Must be a Registered Nurse.
• Demonstrated capacity to provide leadership in articulating BaptistCare's Purpose and Values in management practices and commitment to actively promote the Purpose and Values.
• Knowledge and experience in Aged Care.
• Demonstrated knowledge and understanding of the Australian National Aged Care Classification (AN-ACC).
• Proven experience and knowledge of age care accreditation.
• Experience in training small groups
• Experience and Knowledge with assessment and care planning
