Do you have strong coordination skills and a heart for community impact? LivingHope NDIS Disability Services is seeking a dedicated and compassionate Support Coordinator to join our growing team!

Key Responsibilities : Coordinate and implement NDIS plans to help participants achieve their goals Build strong relationships with participants, families, and service providers Ensure services align with participants' individual needs and goals Navigate supports, resolve service delivery issues, and maintain compliance Maintain accurate records and progress reports What We're Looking For : Experience in NDIS, social work, case management, or disability services Excellent communication, problem-solving, and organisational skills A genuine passion for person-centred practice Relevant qualifications and current NDIS Worker Screening Check Driver's licence and access to own vehicle (preferred) What We Offer : Flexible working arrangements Opportunities for training and professional growth The chance to make a meaningful impact every day Whether you're an experienced Support Coordinator or looking to grow into the role, we'd love to hear from you!
