One should be adaptable and flexible to work with individuals with diverse special needs and to help frail, older people, including the prematurely aged, living in the community to maximise their independence.

- About the Role
- Deivian Care is looking for a Support Worker in Springvale
- with expertise helping vulnerable and marginalized older persons through high-quality case management, social work, and outreach. You will play an important role for many participants who may be excluded from mainstream healthcare, social support, and opportunities, negatively impacting their functioning and independence in the community. You'll use evidence-based, integrated, and person-centered approaches to give individualized assistance using clinical frameworks and modalities to create co-developed realistic recovery programs that are both individualized and powerful. Your goal is to help and encourage people to recognize their own self-efficacy and make long-term changes by taking control of their own journey.
- About You
- A person-centered, values-based approach to service delivery that is informed by a proven commitment to social justice concerns.
- Knowledge and understanding of trauma informed, strengths-based and person-centered care.
- Demonstrated communication and relationship management abilities, as well as the ability to maintain collaborative working relationships with a wide range of stakeholders.
- Job Role and Responsibilities
- Provide support and assistance with living skills, connections with the community, personal and social development, employment, education, and training.
- Assist participants with ADL's, money, health and wellbeing, general life skills, and recreational activities as part of their care management plans.
- As appropriate, create client file notes and assist with administrative responsibilities to ensure that operations function effectively.
- Empower participants to pursue significant goals and make a positive influence in the lives of people.
- Experience / Certification Required
- Relevant qualification in Disability / Age Care / Individual Care / Social Work
- Full Victorian Driver's License and your own vehicle (registered in your name)
- National Criminal Check
- Working with Children Check
- First Aid & CPR (or willingness to get one)
- NDIS Screening Check
- Covid-19 Vaccination Certificate
