The Team : The Tasmanian Health Service delivers the Aged Care Assessment Service (ACAS) South, providing both clinical and non-clinical assessments as part of the Assessment and Case Management Service.
Our dedicated team is committed to delivering high-quality, person-centred care that supports older Tasmanians in accessing appropriate government-funded aged care services.The Role : Within the Tasmanian Health Service framework, a non-clinical Aged Care Assessor conducts Home Support Assessments through the My Aged Care platform.
Using the Integrated Assessment Tool (IAT), the assessor identifies and evaluates an older person's needs, goals, and preferences, focusing on wellness, reablement, and promoting independence.You will need : Well-developed understanding and demonstrated ability to undertake the functions of an Aged Care Assessor, including understanding the needs of older persons and the ability to plan and coordinate client care independently within a multidisciplinary team.Knowledge of the needs of older clients, wellness, reablement, and the use of funded and non-funded services to meet their goals.Strong organizational skills and ability to meet deadlines.Note : Please refer to the Statement of Duties for all the selection criteria of this position to address in your application.Details of appointment : Permanent full-time position, working 76 hours per fortnight, commencing as soon as possible.
Hours are negotiable with the successful applicant.Applicants should note that, for twelve months from the date of publication, this selection process may be used to fill subsequent or similar vacancies, including full-time, part-time, and casual roles.Salary : $81,915 to $94,263 per annum, plus an employer 11.5% superannuation contribution.Salary in accordance with the Public Sector Unions Wages Agreement 2022.Eligibility : Successful applicants must maintain current and valid registration / licences essential for this role.
