We are seeking a motivated experienced Community Development worker with excellent communication skills to join the Complex Care Addiction Medicine Unit who operate within the Community Assessment and Response Team (CART). The Community Development Worker ensures that the patients / clients presenting to hospital with alcohol or drug related issues assists and provides appropriate care that transitions them back into the community. Essential to the role is addiction medicine experience as well as an understanding of the community services available to them. We operate from a person - centred care framework and strongly align with the social model of health.
