About the roleWe are looking for compassionate, reliable, and experienced disability support workers for our clients all over the Perth, Peel and Geraldton region. You can make a difference in our clients' lives by supporting them with daily living activities and domestic duties and empowering them to achieve their life goals.
Working in a team, you will facilitate the delivery of quality services and achieve positive outcomes with service users in a positive and proactive environment. You will be actively supporting their individual needs and creating a safe environment.
- Key Responsibilities
- Support participants to work towards achieving their individual goals.
- Support daily living, promoting cooking, housekeeping, hygiene, shopping, Medication administration, community access etc.
- Support participants in building their confidence.
- Liaise and collaborate with clients, their community and community support agencies.
- Monitor and review the participant's progress through the service and against the support plan.
- Utilise positive behaviour strategies
- Maintenance of progress notes and record keeping
- Support clients to participate in age-appropriate activities and tasks, including employment, in-home support and community access.
- Work in accordance with the medication administration policy, work health and safety procedures and client rights policies, including privacy and confidentiality.
- Monitor the safety and well-being of the clients and report any concerns to the Manager.
- Ensure completion of appropriate documentation. As required, relevant information on support will be recorded in client notes and communication books.
- Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
- Work with clients, family / carers, relevant case managers and other professionals to support the client's individual goals.
- In conjunction with other staff members, assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.
- Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
- Assume the role of key worker for designated clients, including preparing a monthly report on client progress.
- Liaise extensively with other staff to ensure the overall smooth and consistent running of the services.
- Report regularly to the Manager
- Skills & Experience
- Qualification in community / disability / individual support / aged care / Mental Health and preferably with relevant experience.
- Experience working with clients with challenging behaviours.
- Well-developed verbal and written communication skills.
- Current Driver's Licence and reliable vehicle.
- 100-point check ID
- Valid visa / working rights in Australia
- Has as below or willing to obtain before offering :
- First aid / CPR
- Medication cert
- Manual handling cert
- Working with children, check
- NDIS clearance check
- NDIS orientation module cert
- Covid-19 Vaccination evidence (1st, 2nd and booster doses)
