Disability Support Worker

We are looking for compassionate, reliable, and experienced disability support workers for our clients all over the Perth, Peel and Geraldton region. You can make a difference in our clients' lives by supporting them with daily living activities and domestic duties and empowering them to achieve their life goals.

Working in a team, you will facilitate the delivery of quality services and achieve positive outcomes with service users in a positive and proactive environment. You will be actively supporting their individual needs and creating a safe environment.

• Key Responsibilities
• Support participants to work towards achieving their individual goals.
• Support daily living, promoting cooking, housekeeping, hygiene, shopping, Medication administration, community access etc.
• Support participants in building their confidence.
• Liaise and collaborate with clients, their community and community support agencies.
• Monitor and review the participant's progress through the service and against the support plan.
• Utilise positive behaviour strategies
• Maintenance of progress notes and record keeping
• Support clients to participate in age-appropriate activities and tasks, including employment, in-home support and community access.
• Work in accordance with the medication administration policy, work health and safety procedures and client rights policies, including privacy and confidentiality.
• Monitor the safety and well-being of the clients and report any concerns to the Manager.
• Ensure completion of appropriate documentation. As required, relevant information on support will be recorded in client notes and communication books.
• Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
• Work with clients, family / carers, relevant case managers and other professionals to support the client's individual goals.
• In conjunction with other staff members, assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.
• Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
• Assume the role of key worker for designated clients, including preparing a monthly report on client progress.
• Liaise extensively with other staff to ensure the overall smooth and consistent running of the services.
• Report regularly to the Manager
• Skills & Experience
• Qualification in community / disability / individual support / aged care / Mental Health and preferably with relevant experience.
• Experience working with clients with challenging behaviours.
• Well-developed verbal and written communication skills.
• Current Driver's Licence and reliable vehicle.
• 100-point check ID
• Valid visa / working rights in Australia
• Has as below or willing to obtain before offering :
• First aid / CPR

• Manual handling cert
• Working with children, check
• NDIS clearance check
• NDIS orientation module cert
• Covid-19 Vaccination evidence (1st, 2nd and booster doses)
