About the Role

• Inspire and support consumers through tailored education, information, referrals and support to learn the skills and knowledge to build resilience for mental and physical healthcare needs and to engage in their community.
• Model recovery strategies to enable rehabilitation of consumers residing in the Community Care Unit / s (CCUs) while implementing the rehabilitation plans of individual consumers.
• Drawing on mutual resources as peers and utilising a range of recovery tools, techniques and experience, support consumers to identify their own achievable and meaningful recovery and rehabilitation goals, and with planning and support to action the plans, to make positive changes to achieve these.
• Work effectively and collaboratively as a member of the Community Care Unit's multidisciplinary team to provide an integrated model of care, and in accordance with peer values.
