Responsibilities

• Provide specialist symptom and medication management for individuals with life‑limiting illnesses in the community.
• Coordinate care with Social Workers, Occupational Therapists, and multidisciplinary teams.
• Prioritise caseloads, conduct clinical coordination, and maintain strong communication with clients and families.
