The Primary Care and Community Health Unit consists of nine centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non‑government agencies and other organisations to provide care across our centres, in people’s homes, other community venues and via virtual care (telehealth).
St Clair Community Health Centre is seeking an Occupational Therapist to join the team working across our Lemongrove Campus. In this role you will work as part of a collaborative and supportive team of Occupational Therapists, providing services to clients with complex conditions and needs. You will establish short‑ and long‑term goals, complete comprehensive assessments, and implement treatment plans in conjunction with the multi‑disciplinary team. Interventions may involve pressure injury prevention / management, equipment prescription, home modifications, falls prevention and palliative care, among many others.
