About the Opportunity

The Registered Nurse / Child and Family Health Nurse (CFHN) is responsible for the provision of safe, compassionate, quality, client-centred care and works in collaboration with patients, families, carers and a multidisciplinary team to achieve identified goals and outcomes.

The role involves providing universal services to families with children aged 0 – 5 and working in a Sustained Health Home Visiting (SHHV) model. The position collaborates with social work to provide assessment, care coordination and education to families while considering clinical, psychosocial and cultural needs.

The SHHV program works with vulnerable women and their families from the antenatal period, potentially until the child is 2 years of age. It includes home visiting and practical support based on primary health care principles to promote a safe and secure environment.

Responsibilities

Deliver safe, compassionate, culturally responsive care to clients, ensuring effective communication and engagement with consumers, carers, and the multidisciplinary health care team.

Apply critical thinking to provide evidence-based nursing care and participate in risk management, work health and safety and quality improvement initiatives.

Where You'll Be Working

SWSLHD Primary & Community Health provides community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney, including prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care.

  • Current authority to practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA). Child and Family Health nursing qualifications or commitment to undertake and complete in first 12 months, with relevant experience.
  • Demonstrated ability to provide safe, compassionate, quality care that is culturally responsive.
  • Demonstrated ability to effectively communicate and engage with clients / consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
  • Demonstrated evidence of the use of critical thinking as it relates to the delivery of evidence-based nursing care.
  • Demonstrated understanding and participation in risk management, work health and safety and quality improvement in the workplace.
  • Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license from a state other than NSW must obtain a NSW C class license within three months of appointment. Learner's licenses are not permitted.
