About the Opportunity
The Registered Nurse / Child and Family Health Nurse (CFHN) is responsible for the provision of safe, compassionate, quality, client-centred care and works in collaboration with patients, families, carers and a multidisciplinary team to achieve identified goals and outcomes.
The role involves providing universal services to families with children aged 0 – 5 and working in a Sustained Health Home Visiting (SHHV) model. The position collaborates with social work to provide assessment, care coordination and education to families while considering clinical, psychosocial and cultural needs.
The SHHV program works with vulnerable women and their families from the antenatal period, potentially until the child is 2 years of age. It includes home visiting and practical support based on primary health care principles to promote a safe and secure environment.
Responsibilities
Deliver safe, compassionate, culturally responsive care to clients, ensuring effective communication and engagement with consumers, carers, and the multidisciplinary health care team.
Apply critical thinking to provide evidence-based nursing care and participate in risk management, work health and safety and quality improvement initiatives.
Where You'll Be Working
SWSLHD Primary & Community Health provides community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney, including prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care.
