About the Role
We are seeking a compassionate and experienced person with a background in aged care to join our team as a Home Care Packages Case Manager. This role involves supporting elderly clients living at home, ensuring they receive tailored, high‑quality care to maintain their independence and well‑being. As a Case Manager, you will coordinate Home Care Package services, manage client assessments, and create individualised care plans to meet each client’s unique needs. You will work closely with clients, families, and healthcare providers to ensure the best outcomes and enhance the quality of life for our clients.
