As a Care Manager, you will lead and coordinate all client care activities, ensuring a seamless experience for both prospective and existing clients. You will work closely with our office team, community, and families to deliver high‑quality support and meet business goals.
Key Responsibilities
- Build trusting relationships with prospective and existing clients, families, and representative stakeholders.
- Conduct regular quality assurance visits (in person or via phone) to monitor care, address changing needs, and recommend additional services.
- Collaborate with clients, families, caregivers, and other stakeholders to keep care plans aligned with evolving needs and preferences.
- Maintain strong local knowledge and networks by staying informed about community services and engaging with health providers and referral sources.
About You
Tertiary qualification in a health‑related field (social work, allied health, community services) is desirable.Experience in Aged Care or Disability is required.Strong organisational skills; ability to manage time effectively, multitask, and adapt under pressure.Excellent written and verbal communication skills, accurate data entry, proficiency in Microsoft Office.Valid Australian Driver’s Licence and comfortable travel required.Dovida is a national provider of high‑quality person‑centred home care.
