Full Time Care Co-Ordinator - Right at Home Ipswich

About the Role :
As a Care Coordinator , you will be the key link between our clients, their families, and our care team. You will ensure that our clients receive the highest quality home care services by managing schedules, coordinating care plans, and providing compassionate support.
Key Responsibilities :
- Match clients with caregivers based on needs and preferences
- Schedule and coordinate in-home care services
- Communicate with families, caregivers, and healthcare professionals
- Monitor and update client care plans
- Ensure compliance with NDIS standards and aged care best practices
- Provide exceptional customer service and client support
What We’re Looking For :
Certificate III / IV in Individual Support (Ageing and Disability) or equivalent
