The Role :

You will be working as one of two Clinical Care Coordinator s (resident ratio : 1 : 40) and reporting directly to the Resident Services Manager you will be responsible for the delivery of resident focused clinical care services including care planning, review, and assessment of all aspects of clinical care.

Key Responsibilities :

• Ensure comprehensive clinical care is provided, documented, and tailored to each resident's needs
• Investigate incidents daily, particularly Serious Incident Response Scheme reporting and seek continuous improvement opportunities
• Prioritise residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks
• Work collaboratively with Resident services Manager and peers in Implementing Strengthened Aged Care Quality Standards
• Oversee and conduct all scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required
• Lead the clinical team including RNs and Ens for daily case reporting, care planning, and addressing patient concerns promptly
• Fortnightly report to Clinical and Quality Committee Meeting on care quality, SIRS and clinical risks
• Report quality indicators
