Clinical Care Coordinator (Grade 5)

The Role

Reporting directly to the Resident Services Manager, you will be responsible for the delivery of resident-focused clinical care services including care planning, review, and assessment of all aspects of clinical care.

Key Responsibilities

• Ensure comprehensive clinical and specialized nursing care is provided, documented, and tailored to each resident's needs by our clinical staff
• Prioritise residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks
• Oversee and conduct all scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required
• Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly
• Conduct routine clinical assessments to promptly address identified gaps in care
