HammondCare - Home Care is seeking an experienced Community Nurse - Aged care Partner to support the team in Melbourne, covering the south and east regions.

Essence of the role

The Community Nurse - Aged care , internally known as Clinical Partner , delivers high-quality, person-centered clinical care that supports clients to live well in their own home, in line with HammondCare’s Mission in Action.

• The role provides evidence-based, best-practice clinical care holistically, to individuals, ensuring their goals, needs and preferences are understood and addressed.
• Focus on assessing clinical care needs, developing and reviewing care plans, and delivering excellent clinical care in partnership with clients and key stakeholders.
• Collaborates with the care management team to achieve the best outcomes in the community setting and guides care staff to ensure the safe and continuous delivery of high-quality care.
