Responsibilities
- Deliver comprehensive case management and tenancy support guided by Australian Housing First Principles to achieve long-term housing stability.
- Provide holistic, strength-based support and intensive, flexible assistance for young people, single adults, and families with complex needs and increased vulnerabilities (e.g., trauma, AOD).
- Work effectively in a multidisciplinary team to conduct culturally safe care coordination, service navigation, and referral to mainstream and specialist services.
- Employ Trauma‑Informed Care and Practice to support clients in obtaining and sustaining housing, using flexible brokerage to address their individual needs and goals.
- Actively participate in community networking and convene multi‑disciplinary team meetings to ensure an integrated, effective support system for clients.

Extensive experience providing holistic, strength-based, trauma-informed case management to vulnerable individuals and families with multiple and complex needs.Demonstrated knowledge of the contemporary homelessness context, Australian Housing First Principles, and the ability to apply this to determine and deliver effective, consumer‑led housing solutions.A strong commitment to culturally sensitive practices, effective service delivery, and the capacity to build strong links within relevant community networks (especially in Gippsland).
