Responsibilities

• Deliver comprehensive case management and tenancy support guided by Australian Housing First Principles to achieve long-term housing stability.
• Provide holistic, strength-based support and intensive, flexible assistance for young people, single adults, and families with complex needs and increased vulnerabilities (e.g., trauma, AOD).
• Work effectively in a multidisciplinary team to conduct culturally safe care coordination, service navigation, and referral to mainstream and specialist services.
• Employ Trauma‑Informed Care and Practice to support clients in obtaining and sustaining housing, using flexible brokerage to address their individual needs and goals.
• Actively participate in community networking and convene multi‑disciplinary team meetings to ensure an integrated, effective support system for clients.
