About the role
Ballycara is seeking a Social Worker to join the team!
Reporting to the Senior Social Worker, this role will support the social & mental health of clients by utilising professional expertise to guide and inform the case management of individuals with multi-faceted / complex issues across a variety of programs.
- Skills and Experiences
- The successful applicant will hold
- Degree in Social Work from an accredited course and membership with the Australian Association of Social Workers
- Current Driver's Licence and reliable car with willingness to use for work purpose travel. (KM will be paid between clients)
- Clinical skills in assessment, delivering appropriate interventions, counselling, advocacy and liaison and referral to ensure access to appropriate community services
- Experience working with older adult clients who have chronic and complex health conditions and the capacity to work with this client group in a community setting under various MyAged Care funding models (CHSP, HCP, STRC)
- Experience / knowledge of MyAged care system
- Understanding of the psychosocial impact and management of chronic health conditions, ageing and disability and knowledge of grief and bereavement theory,
- Ability to work independently at a professional level as well as the ability to work productively as a member of a multidisciplinary team in a community health setting
- To work as part of a muti-diciplinary team to ensure the development and delivery of effective and pro-active social work services
- Demonstrated commitment to person-centred care
