About the role

We are seeking a passionate and experienced Support Coordinator to join our team at Supports For ME Pty Ltd on the Sunshine Coast, Queensland. As a casual Support Coordinator, you will play a vital role in assisting our NDIS clients with their needs to access the services and support they require to live independently and with dignity.

What you'll be doing Coordinate and monitor the implementation of client NDIS support plans, ensuring all services are delivered as per their individual requirements and assist them to maintain these supports in order to achieve their goals

Liaise with clients, their families, and a wide range of service providers to facilitate the delivery of holistic, person-centred care

Liaise on behalf of clients to ensure their needs are understood and met by support services

Regularly review client progress and make adjustments to support plans as required

Maintain detailed case notes and records in accordance with organisational and legislative requirements

Participate in team meetings and contribute to the continuous improvement of our service delivery

Provide care coordination supports to the wider team and ensuring required documentation is completed in line with legislation requirements for NDIS participants

Engaging in local network and stakeholder meetings to stay informed of services available and promote our organisation

What we're looking for Previous experience as a Support Coordinator is essential

Qualifications in disability services, mental health or community work highly desirable

Experience in case management, reporting requirements for NDIS and stakeholder liaison is key

Excellent communication and interpersonal skills, with the ability to build rapport and advocate effectively on behalf of clients

Strong organisational skills and attention to detail to ensure accurate record-keeping and timely service delivery

Demonstrated understanding of the NDIS and the ability to navigate complex funding, service arrangements in this evolving environment

Empathy, patience, and a genuine commitment to improving the lives of people with disabilities
