Our Care Consultants have experience with Case Management / Care Plans. You'll be visiting our clients in their homes : located in the North-West suburbs of Metro Melbourne, to do the following :
Initial Customer assessments & service planning
Work with your client to understand what they want from their Home Care Package, and then help them to build it based on their level of funding
Care planning and regular monitoring and reassessment of customer needs
Providing ongoing case management to HCP and or private customers to meet service and care plan goals
You'll be in the field 70-80% of the day, visiting clients. You'll have the option to work from home in between visits for report writing and case notes.

Qualification in Case Management, or
Undergrad qualification in Nursing / Allied Health / Social Work is essential
Div 1 Nursing Qualification - desirable but not essential
Prior case management experience will be highly regarded
Experience completing initial and annual customer assessments and care planning
Knowledge of the Home Care Package industry
Must be willing to have a flu vaccine and undertake a police check
Must have Covid-19 Vaccinations
