Case Manager

Apollo Care Services is a provider of care services for both the Aged and Disabled. We are community driven and our goal is to provide holistic care and empower those who are less fortunate with both the education and tools to have a higher quality of life.

We are looking for an enthusiastic Khmer Case Manager to further develop and grow our business whilst also empowering our broader local community.

• Responsibilities
• Assist in-home assessments, referrals, care plan reviews & goal planning for consumers
• Assist client budget & reporting
• Excellent time management with client case load
• Respond & communicate effectively with clients re : changing care needs
• Possess empathy & understanding towards the needs of older people
• Working within a broader team and liaising with our expert staff

• Requirements for Success
• Knowledge of & enthusiasm for Disability and Aged Care Services & a Consumer Directed care environment
• Diploma level or above qualification in community, aged, health or another discipline desirable
• Access to a reliable, registered motor vehicle with full comprehensive insurance and a valid driver's license
• Strong computer skills
• A clear national police check and working with children check upon role offer
