We are looking for an enthusiastic Khmer Case Manager to further develop and grow our business whilst also empowering our broader local community.
- Responsibilities
- Assist in-home assessments, referrals, care plan reviews & goal planning for consumers
- Assist client budget & reporting
- Excellent time management with client case load
- Respond & communicate effectively with clients re : changing care needs
- Possess empathy & understanding towards the needs of older people
- Working within a broader team and liaising with our expert staff

- Requirements for Success
- Knowledge of & enthusiasm for Disability and Aged Care Services & a Consumer Directed care environment
- Diploma level or above qualification in community, aged, health or another discipline desirable
- Access to a reliable, registered motor vehicle with full comprehensive insurance and a valid driver's license
- Strong computer skills
- A clear national police check and working with children check upon role offer
