Join a motivated and experienced team and deliver quality intake and assessment for the CommunityWhy you'll love the roleAs our Community Intake and Assessment Officer you will be responsible for Community Service Intake and Assessment of clients entering our services and requiring supports either via My Aged Care and the Commonwealth Home Support Program (CHSP) and Home and Community Care Younger Persons Program (HACCPYP) Programs. In this position you'll be responsible for the assessment and coordination of HACCPYP eligible clients to be linked to appropriate community supports.
You'll be part of the Aged and Disability Services team that undertakes planning, policy, advocacy, partnership and service delivery roles in providing quality home and community support services for older people, and people with a disability and their support and in the development of strategic plans for our older community.
- What you'll do
- Ensure all referrals via My Aged Care are logged onto the Banyule operating system and allocated to waiting list or services.
- Provide input into the overall planning, policy development and co-ordination of related Aged and Community Support Programs.
- Develop and maintain constructive working relationships with appropriate local and regional service providers.
- Provide advice on current service trends & assist the development of new programs in areas of need
- Provide a high-quality customer focused service, providing opportunities for consumer participation and feedback regarding the quality-of-service delivery, planning and policy development
- What you'll bring / what we are looking for
- Tertiary Qualifications and relevant experience in aged or health sector
- Demonstrated understanding of the needs and issues facing older people, people with disabilities and their carers
- Demonstrated experience in the undertaking of holistic assessments and care planning.
- Experience, knowledge and understanding of the CHSP program and HACCPYP program.
- Knowledge and understanding of the Aged Care Reforms
- Understanding of the Wellness and Reablement approach to services, including support planning and review.
- High level of competence in computer bases systems
- Strong understanding of OHS requirements for clients and staff and able to offer guidance and resolution.

- Nationally Coordinated Criminal History Check (NCCHC) (Previously known as a police check)
- Working with Children's Check
- CPR / First Aid Certificate
- We offer
- Salary range $75,580.79 to $82,169.90 per annum plus super (for comparison at full time this would be $94,475.99 to $102,712.38 per annum plus super)
- Temporary (to 27 January 2027) part-time 30.4 hours per week (0.8 Equivalent Full Time)
