Corryong HealthAustraliaSocial Worker Corryong Health (CH) is a public sector agency and one of six multi-purpose services in Victoria. Corryong Health is funded by Commonwealth and Victorian State Government funding, and provides a broad range of hospital, residential aged care, primary health, and community support services.
Position Objective / About the Role : The Social Worker is responsible for providing social work activities in response to referrals from the Corryong Health Multidisciplinary team. The person will be required to have a strong interest in working with older people, children with special needs, and their families. The SW must be motivated to take initiative in implementing relevant projects to meet community needs and ensure that Corryong Health services are positively promoted in the community.
Position Specific Requirements : Provide social work support for individuals and families within a multidisciplinary setting Work across community, acute, and residential aged care settings Collaborate with all members of Corryong Health Team to ensure holistic care Offer services for families and carers that include flexible and tailored responses and support groups Support clients experiencing family violence and connect them with specialist services Provide case management to relevant clients Participate in multidisciplinary team programs, including continuum of care, and carry out case management as required Undertake individual client assessments using agency tools Collect relevant data for monthly reporting requirements Maintain accurate client records and contribute to multidisciplinary care plans Keep the Allied Health Manager informed through regular meetings Participate in regular supervision with an external supervisor (supported through contractual arrangements) Work with clients to address ongoing needs Selection criteria Essential : Bachelor of Social Work or relevant tertiary qualification Membership with the Australian Association of Certified Social Workers Relevant postgraduate experience in Social Work Desirable : Three years of experience working as a social worker Why choose Corryong Health?
