Social Worker | Part Time - 2 days per week | $85,000 base (pro rata) + superannuation | Veterans & Community Health

We are seeking an enthusiastic and compassionate Social Worker to join our multidisciplinary team supporting Veterans and the wider community.

Starting in early to mid-November 2025, this part‑time position (2 days per week) is based at our Frankston clinic, with opportunities for additional Telehealth and future expansion into new sites.

You will provide high‑quality social work support to a Veteran‑focused caseload, while also working with adult community patients referred through our clinics. Initially managing a smaller caseload, you will have the opportunity to grow your caseload as demand increases.

This is a generalist social work role, involving health coordination, referrals, and connecting patients with appropriate resources. You’ll work across both clinic and community settings, supported by a collaborative multidisciplinary team including Exercise Physiologists, Physiotherapists, Social Workers, Dietitians, and Podiatrists.
