Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District. PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community. PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services. The Role
The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional care, healthier lives’. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.
SESLHD Drug and Alcohol Service offers a range of treatment and support services for people with problems from their alcohol, prescription or illicit drug use, and / or their families and carers. Outpatient D&A services are located in clinics at the Langton Centre in Surry Hills, St George Hospital in Kogarah, and Caringbah Community Health Centre at Sutherland Hospital. Inpatient D&A services are provided at Sydney Hospital, Prince of Wales, St. George and Sutherland Hospitals.
Our goal is to improve health, wellbeing and social outcomes for individuals, families and the community, related to drug and alcohol use.
The Clinical Nurse Consultant is a Registered Nurse who has responsibility for working collaboratively with Drug & Alcohol Services and the Mental Health Services to assist in the provision of comprehensive care to people with co-occurring substance use and mental health conditions (“comorbidity”). Key responsibilities of the position include the provision of comorbidity nursing consultancy and liaison services to inpatients and community clients of the Mental Health Services and Drug and Alcohol Services, as well as carers and health professionals across the Mental Health Services.
The position delivers clinical leadership and education, and participates in research and clinical services planning in line with the criteria for a Clinical Nurse Consultant Grade 1 as per the NSW Public Health System Nurses and Midwives (State) Award.
