Your new role

As a FPR Practitioner, you will provide intensive wraparound case management services to vulnerable families who present with complex needs. You will deliver assertive outreach, undertake risk assessments, participate in care team planning and develop creative interventions to engage hard‑to‑reach families. Working as part of a collaborative care team, you will assist families to address the safety and developmental needs of children and young people, and support parents to enhance their resilience and self‑determination.
