The Salvation Army (Salvos) is one of the largest providers of homelessness services across the country. Our philosophy is that every person should be afforded dignity and respect and that no one should be without a safe, affordable, and secure home. We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

About The Role

As a Case Manager, this is your opportunity to actively engage with people who are rough sleeping and who may be disconnected from mainstream services and supports. This dual role combines Homeless Outreach Program Case Manager and Care Finder – providing support, advocacy, and service connections for people experiencing homelessness and older adults needing help accessing aged care.

Reporting to the Team Leader, this is a full‑time role on a fixed term and contracted until October 2026, located in Nunawading, VIC.

How You Will Make An Impact

• Provide holistic case management and information services aligned with the National Homelessness Stream Model of Care.
• Conduct assessments, develop case plans, offer interim support, and manage case closures.
• Engage clients to address housing and support needs, promote self‑determination, and navigate service systems.
• Build collaborative relationships and advocate for client needs while ensuring inclusive, culturally safe, and LGBTIQA+ affirming practices.
• Maintain accurate records, meet reporting requirements, and actively participate in training, supervision, and team development.
