Women's House Case Manager

Your new role

Women’s House is an initial point of contact for women, trans and gender diverse folks experiencing or at risk of homelessness. We strive towards offering a safe and supportive space that offers brief intervention housing support as well as showers, laundry, hot meals and social inclusion. We interpret individual’s experiences within a systematic framework that acknowledges external factors that perpetuate disadvantage; we commit to advocacy and action towards a more equitable and just society for the women, trans and gender diverse folks we support.

Responsibilities

• Utilising persistent and assertive engagement techniques to establish and maintain a professional relationship with service participants.
• Hold a caseload of 12 participants for up to 6 months.
• Coordinate case conferences, planning and exit planning in conjunction with referring agencies.
• Develop and foster positive relationships with local services and agencies.
• Monitor, progress, and achieve program targets and outcomes.
