Uniting Communities are seeking a passionate and highly motivated
- Team Leader
- for the Staying at Home Program. The 3 day Staying at Home Program aims to empower carers with the knowledge, skills and capability to support and navigate their caring role for someone living with dementia. The program aims to demystify dementia and empower carers to view respite as a chance to step away from their regular caring duties, helping to sustain the caring relationship for as long as possible. Over three days and two nights the guests living with dementia are supported to connect to their peers and engage in discussions regarding the impact of their diagnosis in a safe space.
The SAHP Team Leader will be responsible for the delivery of this program across metro and regional South Australia, supporting retreats to people living with dementia and their carers. An exciting opportunity to join our Home and Community Aged Care team in Adelaide, where you will work closely with the Leadership team to ensure that our customers receive high quality individualised services that meet their needs.
This role will be best suited to an individual with Allied Heath (eg, Social Work, OT) background._Duties and responsibilities of this role include, but are not limited toProvide operational oversight of the Staying at Home Program across metro and regional South Australia, ensuring that retreats are being delivered in line with program guidelines and Activity WorkplanProvide hands on supports to all scheduled retreatsIntrastate travel required across regional South Australia. Overnight stays maybe requiredProvide supervision and support to staff facilitating the programEnsure that program feedback and evaluation is completed and program improvements identified are actionedPromote the program with key stakeholders and support the tailored delivery of services to clients groups including First Nations people, people from Culturally and Linguistically Diverse backgrounds and people from the LGBTIQA+ community to ensure that retreats are culturally safe and appropriateClinical experience and knowledge of dementia and its impacts on people living with dementia and their carers (highly desirable)Professional experience in the delivery of community based and overnight support servicesFamiliarity with Aged Care StandardsDemonstrated experience in coordination and administrationExcellent communication skills with a focus on high quality customer serviceUnderstanding of funding requirements to ensure compliance is metAbout UsUniting Communities is a leading, inclusive not-for profit community services organisation working alongside more than 80,000 South Australians each year to reduce inequality, improve wellbeing, overcome disadvantage, and support people to live the best lives they can.
