Mungabareena Aboriginal Corporation is seeking a dedicated Pathways to Good Health Navigator.
The Pathways to Good Health Navigator role is essential in ensuring Aboriginal children and young people entering care receive timely and priority access to health assessments, including age-appropriate screenings, '715' health checks, and maternal & child health assessments etc.
Navigators will work closely with both children and their carers to guide them, linking them to internal services within the Local ACCO and external providers via the partnerships established by the Coordinator and Mungabareena Aboriginal Corporation.
In this key role, you will : Offer culturally safe and responsive services seamlessly by clarifying health and wellbeing processes for children and carers, reducing fear and uncertainty, and ensuring they understand how to access the services they need.
Identify unmet health and wellbeing needs, identify and navigate barriers to engagement by offering a flexible model of care.
Assisting in coordinating referrals and follow-ups, ensuring children have continuous care and access to relevant specialists or support services.
Develop health and wellbeing management plans, co-ordinate client care in collaboration with the multidisciplinary team. This includes participation in case consultations, care team meetings with external partners involved in client's care, monitoring client progress and ensuing review.
Serving as a liaison between carers, children, and healthcare providers, ensuring that appointments, care plans, and health assessments are properly understood and executed.
Collaborating with other Mungabareena Aboriginal Corporation team members and external services to ensure that every child's health needs are met in a timely and comprehensive manner.
Navigators play a key role in ensuring that the services provided not only meet medical needs but are delivered in a culturally safe, responsive, and supportive manner, empowering families to access appropriate health services while navigating the often-complex care system with a social, emotional and wellbeing focussed approach to implement the services and support in a holistic approach.
