Job summary CareNavigator - 2 x part time / 1 full time availableResponsible to Reception Manager / Operations ManagerOur Care Navigators provide the first point of contact for our patients and act as a focal point of communication between patients, doctors, and other medical staff.Care Navigators are key members of The Quarter Jack Surgery team.
They are trained to work with all available resources, which may include signposting patients to other services, offering self-help advice, or booking appointments with appropriate clinicians in the practice's multidisciplinary team.
This work is often referred to as care navigation.The following are the core responsibilities of a Care Navigator.
There may occasionally be a requirement to carry out other tasks; this will depend on workload and staffing levels.
