We have an exciting opportunity to work as a Support Worker in D&J Connect support services team. As a support worker you'll provide a range of support interventions within a developmental framework that enables NDIS participants to address their needs. You'll assist in delivering activity based work and daily living supports such as taking participants to appointments.

Position Title Mentor (Disability / Psychosocial)
Salary $29.12 per hour plus 10% Superannuation
Contract Period 12 months maximum term contract. The role is conditional on the continuation of viable funding under the NDIS

Certificate III in Individual Support (Disability)
As a condition of employment, the following are required :
Police check A criminal record check completed within the last 6 months must be supplied by all new appointments. Where a new employee has lived outside of Australia for 12 months or more within the last 10 years, an international check is also required.
Working with Children check A valid Working with Children check must be supplied by all new employees.
Car licence A valid Australian driver's licence (C Class or above) must be supplied by all new employees.
Suitable Vehicle A roadworthy vehicle insured to a minimum level covering 3rd party fire and theft
comprehensive cover is recommended).
The vehicle must be capable of transporting multiple passengers in a safe manner.
Right to work within Australia Australian or New Zealand citizenship or relevant working visa documentation.
Position Context
The introduction of the National Disability Insurance Scheme (NDIS) marks a significant change in how disability services are funded, moving away from a black funding model and introducing a competitive market for disability services. At D&J Connect our support coordinators will work with our participants to understand their NDIS plan and assist them to build capacity within themselves to achieve their NDIS goals. The support coordinator will assist you to understand and implement the funded supports in your plan and link you to community, mainstream and other government services. The role is conditional on the continuation of viable funding under the NDIS.
Position responsibilities
The Support Worker (Disability / Psychosocial) is responsible for the following :
Accompanies people with a disability to medical / specialist appointments.
Support participants to participate in age
Support participants to access activities and All participants receive a high level of care outings in the community. and support as per their NDIS plan and goals.
Provide direct care assistance to each Personal care is provided to participants in participant in accordance with their NDIS plan a caring and supportive way and as directed by D&J Connect Support Coordinator.
Work in accordance with the medication Documented evidence demonstrates that administration policy, work health and safety support services are followed. procedures and participants right's policies including privacy and confidentiality.
Monitor the safety and well-being of the All policies and procedures are followed. participants and report any concerns to the
D&J Connect.
Provide 1 : 1 or small group training and support All policies and procedures are followed. in response to individual participant needs.
Ensure completion of appropriate All policies and procedures are followed. documentation. Relevant information on support provided is to be recorded in client notes and communication book as required.
Review each participant NDIS plan and Records demonstrate that clients have monitor outcomes. received correct medical and dietary intakes as prescribed by medical practitioners and as per TDT policy and procedures.
