PlanCare Holdings Pty Ltd is seeking a Co-Care Partner (Case Coordinator) to oversee and coordinate the delivery of aged care home care services across Australia. This role involves assessing client needs, managing home care support plans, liaising with stakeholders, and ensuring high-quality service delivery in line with the Aged Care Act and Home Care Package funding.
- Key Responsibilities
- Coordinate and oversee the delivery of aged care home care services for clients across Australia.
- Conduct face-to-face meetings and onboard new clients, ensuring a smooth transition into services.
- Manage rostering, sourcing, and arranging client support services, including after-hours support as needed.
- Maintain accurate and confidential client records and conduct regular support plan reviews.
- Advocate for clients, liaise with medical professionals, allied health services, and stakeholders to ensure optimal care.
- Monitor and evaluate service effectiveness, ensuring compliance with industry regulations.
- Develop key partnerships with industry contacts to promote and expand PlanCare's services.
- Provide feedback to improve PlanCare's products and services in line with client needs.
- Main Tasks
- Assess individual client needs and develop tailored care plans.
- Coordinate referrals to health, welfare, and aged care services.
- Engage in community outreach and relationship-building activities to enhance service accessibility.
- Ensure compliance with aged care funding requirements, government regulations, and company policies.
- Prepare reports on client progress and service effectiveness.
- Participate in quality improvement and accreditation programs to uphold service standards.
- Relevant Qualifications
- Diploma in Community Services, Social Work, or a related field (or equivalent experience).
- In some cases, extensive experience in community services or aged care may substitute formal qualifications.
- Skills & Experience Required
- Minimum one year of experience in aged care management, case coordination, or home care support services.
- Broad knowledge of the Aged Care Act, Home Care Package funding, and Accreditation Standards.
- Strong interpersonal and communication skills, with the ability to work within a team.
- Excellent time management and problem-solving abilities.
- Proficiency in client assessments, care coordination, and service implementation.
- Ability to work under pressure, manage multiple tasks, and adapt to changing priorities.
- Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint) and case management systems.
- Essential Requirements
- Police Clearance Check or NDIS Worker Screening Check (or willingness to obtain).
- Right to live and work in Australia.

Pay : Up to $79,000.00 per year
