Pivot Health Group is Hiring! We are excited to hire a Social Worker to complement our multi-disciplinary team composed of Social Workers, Exercise Physiologists, Physiotherapists, Dietitians, Podiatrists, and GPs(Soon to come). We have been working on a distinctly unique model. The Social Worker plays a vital role within the allied health network in assisting clients transition through their health journeys, interacting with a wide range of health professionals based at the clinic / s and health centres. We are also proud to service a large cohort of Veterans in their health journey.
Social work with the client group focuses on the preservation and / or enhancement of psychological health and wellbeing, social functioning and quality of life.
Key responsibilities of your role as a Social Worker include :
- To provide Social Work services to eligible clients, to be carried out within the relevant framework and policy
- Delivering evidence-based assessment and treatment to patients
- Undertake assessments, case formulation, treatment planning, case management
- Health coordination for clients
- Assess and conduct goal setting for clients
- Psychosocial assessments
- Conduct patient case reviews with other clinic based service practitioners
- Conduct internal and external referrals
- Develop and deliver social work based programs
- To provide reports and undertake recording to the clinical standards and required
- Building and maintaining rapport with patients and other allied health professionals
- Communicating and reporting to other health and medical professionals
- Provide and facilitate health education and promotion for patients
- General wellbeing checks on clients
- Provide generalist counselling to address emotional, behavioural, and mental health challenges
- To provide appropriate support to veterans as per DVA guidance
- Intervention and treatment planning for psychosocial issues and rehabilitation from injury or illness
- Obtain consent for social work services
- Engage with clinic patients or gym members to promote social work services and provide program education
- Facilitate community development initiatives, mobilising resources and organising programs to address social issues and build stronger communities
- Facilitate social events for clients to engage in for mental and physical wellbeing
- Qualifications
- A minimum of 2 year of experience would be required.
- Provide social work clinical services in accordance with the Australian Association of Social Workers (AASW) Code of Ethics, AASW professional practice and Pivot Health Group Practice Guidelines to achieve positive outcomes in meeting client's needs.
- Demonstrated skill, knowledge and expertise in social work practice, theoretical perspectives, social systems, legislation and policies.
- Ability to use evidence-based practice for decisions and actively maintain professional skill development.
- Ability to communicate with people at varying levels to build and maintain positive relationships with the leadership team, managers, team leaders and employees.
- Demonstrated ability to work cooperatively and communicate effectively using well developed oral and written communication skills to enable effective interaction with consumers, carers, and other stakeholders.
- Ability to set realistic work goals, establish work priorities and work autonomously to deliver timely and efficient Social Work services.
- Ability to prioritise tasks, demonstrate initiative and perform to deadlines in accordance with set policy and procedures.
- Demonstrated commitment to participation in relevant professional development, research and continuous learning.
- Actively participate in the service arrangements of the Social Work Division including team meetings and data collection requirements.
- Self-motivated who models professionalism and tact.
- Strong presentation, facilitation and writing skills.
- Strong knowledge of MS Office programs including Word and Excel and ability to learn office specific software.
- Interpersonal skills required for working in teams and managing potential conflict.
Pay : $74,000.00 - $85,000.00 per year
