About the role

The permanent part-time (4 days / week) role of the Ambulatory Service Coordinator is to work within the Mission and Mission in Action of HammondCare to lead and develop a high quality, efficient and effective ambulatory care rehabilitation service. The Ambulatory Service Coordinator will also provide an effective liaison role between internal and external stakeholders, including patients and their carers, facilitating understanding and relationship‑based care.

Key Focus Areas

• To lead the planning, management, and delivery of ambulatory care rehabilitation services including allied health, medical and administration team members in a manner that supports the provision of effective and efficient rehabilitation services to patients and their carers.
• Participate as a member of the Healthcare and Hospitals team in the application of the organisation’s Mission and values to the pursuit of its objectives in a patient‑centred, continuous quality improvement work environment.
• Advocate for Healthcare and Hospitals ambulatory rehabilitation services by openly promoting service capability and capacity.
• Proactively and creatively cultivate relationships and develop strategic opportunities to grow ambulatory services.
