About the role
Based at VincentCare’s Northern Community Hub, this dual role supports young people aged 17–25 who are linked with the youth justice system and are at risk of homelessness. The position combines responsibilities across the Youth Justice Community Support Services (YJCSS) and Youth Justice Homelessness Assistance (YJHA) programs.
You will work collaboratively with partner agencies to provide holistic, goal-focused case management through an outreach model, supporting young people to access safe housing, education, employment, and specialist services addressing issues such as mental health, substance use, family violence, and justice involvement.
As a Senior Case Manager, you will also lead complex casework, coordinate service responses, and provide operational leadership within the Youth Justice and Support Services Team, applying specialist knowledge and professional judgement to achieve positive outcomes for young people.
